Tbh really, the country hex codes are just the country IDs, only they start from 00 (instead of 11) and are in hexadecimal. You can find the country IDs in the stringtable.

As for the hex country codes, the only way to find them is to calculate them by yourself using what I told you earlier. Th problem is, these codes differ from each conquest (because some countries do not exist in certain conquests). Here is a list of them though (for 1939/1950):

And for the color codes… I don't know. They match to hex code of the color used in their borders. You will have to find @communist's list for that.
