I think to unlock the promotion you need to finish normal allies and axis campaign for Europe. This is because I checked the first pacific lvls and the 1943 conquest and there the generals have green bar and the bronze medal showing they been promoted. While in 1939 conquest and Europe campaign all generals I seen only have blue bar.

The Konev mission is quite hard tho. What I did was deployed 7 howitzers and attacked the city at west. Then after captured it, Manstein will immediately come down for ya. So flanked him with all the artillery you have then you can knock him. However, u need to upgrade your artillery in HQ for a certain amount of good damage.
