Medals can be used for promoting Generals, raising skill levels or buying new generals, and buying items in the store.
Money has several similar uses (raise title, training, buying items, etc.)
How do you decide when to do which?

That depends on what upgrade is the most important at the moment and how much you have of one resource.
For medals I'd say:
A good starter general (preferably Ney. Dabrowski or Murat if you can do it) = Ambulance #1 > Other important generals > Healing items > Important general skill upgrades > attack stat items > Gold Medals > General HP upgrades > Bad general skill upgrades > Other items
Coins you'll mostly just spend on tech because most of the coin items are garbage.
So I'd prioritize it like this:
Cav tech > Inf tech > Art tech > Title upgrades > Standard short swords and Commanding Knives if someone can benefit from them > Fort tech > Fence, bunker, trench tech
