In my first run, when I took the bonus city, I used the reinforcements to capture the island base. However, Octavian managed to survive and my men were stretched thin trying to suppress enemy resistance
In my second run, I deployed the reinforcements on land, but couldn't defeat the island gens fast enough.
Any tips?

There is another help request thread, but no guides.Taking the coastal lvl 5 target gives you control over the pirates and even pirate reinforcements, IIRC. Use these troops to take the island and spam slingers to handle Octavian's elites. Octavian is hiding in the NE corner city. Use plague and fire to wear down his troops and guards' HP. It's a bloody fight to the finish so you may need more gens and/or higher lvl skills on your gens. Upgrade your unit tech to have better stats as well. Both will help you get a 2* clear.

The timed condition for the 3rd * is the one I still struggle with.

When you attack the center city (IV), the "key unit cav general" will make his way into the bottom left city, recapture it, and stay there. Melt him down slowly w/ slinger spam and don't waste your main troops on him. Take your time. (ii) When you take the level V city, replenish units and capture everything on mainland with your main force. Leave Taormina. (iii) ALL pirate reinforcements should go for the island. No generals needed, they will eventually overwhelm the enemy. If you follow this strategy, your main force should be quite intact when you face Octavian, you can defeat him under 16 turns.

Dunno if you finished a 3 star on this one but i managed to do it.
Generals deployment: Mithridates on javelineer Anthony on heavy cav (the one near Pompey jr.) Huo on heavy cav Crassus on heavy inf Octavian on bowman
1.) With Pompey jr. atack lvl 3 city next to him; rush Anthony and Mithridates to lvl 5 city to gain acces to pirates fleet. Left group ( Octavian, Huo and Crassus ) capture nearest lvl 3 city.
2.) After capturing lvl 5 city you will aquire pirate fleet under your control. You will gradually get pirate reinforcements. Focus on the island city and wipe all resistance with pirate fleet. Hold Crassus in the lvl 3 city just captured south west to defent against javelineer and push with Huo and Octavian and Pompey jr. to get lvl 4 city in the middle of the continent. Defend coastal lvl 5 city at all costs. Note: after you capture lvl 5 city you will be on crazy beligerance so you need to calculate your evey move since you cannot replenish troops for a while.
3.) After you stabilize your defence in all citys you should be on halfway of conquering island city and beligerance level drop to agressive/defencive. Replenish 1/2 generals in lvl 5 city and go north on coastal to get lvl 3 city. Note: to defend citys it;s enough if you spam light inf.
4.) By the time you get lvl 3 coastal city north you should be finished with city island. Push all forces to Taormina and kill Octavian.
PS: Spam forced march and use pleague/arison/destroy in Taormina.
Try few times. You should get it eventualy.
