I have enough medals to get Johnston, and nearly enough to get Stepanovich. I'm leaning toward Stepanovich simply because I can use him in history mode. It seems both need some textbooks to straighten up. Johnston, 10 textbooks, Stepanovich, 15. Is the history aspect a solid reason to to spend 100 more medals? Thoughts from this outstanding community are welcome.

Johnston will also need 15 text book (2 skill change). Even you haven't yet change his skill, he still has 3 great skill (Edge, precision, and Tactic Master) and can reach King. Stepanovich only has 2 (precision and tactic master or 3 if you like cover) and he only reach prince. Also that 2nd balkan war is really hard so just buy Johnston first, you can still buy stepanovich later

If you equip them both with browning pistol, their max extra attack % are the same. The max extra defense for Johnston is 4% higher than Stepanovic so you can say Johnston is slightly better. However, Stepanovic is needed in 2nd Balkan War whereas Johnston is not needed in any historical campaigns. I bought Stepanovic after Plumer and used him as the no. 3 cav gen. I only bought Johnston when I started tackling challenge conquests (he wasn't used in campaigns).
Stepanovic requires more books and Johnston needs more silver medals so the difference isn't that big and books can be farmed more easily than silver medals.
2nd Balkan War is not difficult if you have the full team. The problem is that Micis requires 530 medals. Given that the bonus item Top Secrets enables you to get at least 1 more mission, I think it's worth it.

I guess I need to just take another look at just how difficult history mode is going to be. I haven't even tried it in a couple weeks. I almost 3 starred Gettysburg with Sherman, McClellan, Meade and Sheridan, But Lee was passing around criticals in the last round, like a joint, and I lost one too many generals killing him. And that was before I even had any tents or wagons. Y'all think Beseler and Moltke are enough to get through Prussian history? If not is Falkenhausen the answer to that? Just opened up chapter 7 and Moltke maxed out at Marquess, with Jungle added seems super squishy to me. It a full time job trying to keep him healthy. Guess that's why I'm looking for more Cav. Could try to upgrade Sheridan, but that seems like a waste of resources.

Also, I got MacMahon, instead of Straussenberg, because A: I thought he would be useful in History mode, and B: it saved me a lot of medals. I changed Counterstrike to Mountain and that's working well while I get him up speed. But Cover seems redundant with Ambush. What would y'all change Cover to? Defence Formation? I'm working him in tandem with MacArthur.

To 3-star Battle of Gettysburg, I used double attack items at start of every turn and swapping to double healing items at end of every turn plus disbanding near dead gens. I also put Farragut on art (instead of Miles) so he could heal every turn (make sure you put Officer Sword, Cross Sword or Standard Short Sword on him as the increase in art ability part of art quadrant does not have effect on non-art gen).
You should be able to 3-star AP War easily with Beseler, Moltke and Falkenhausen. That's how I started using AP war to farm book/silver medal.
Upgrading Sheridan (or any other gen in the US team) is definitely a waste of resources.
Straussenberg is my first medal inf gen (MacMahon and Maria my second and third). In hind sight, I should use Garibaldi and not buy Straussenberg at all. Inf gens are not important in this game especially in challenge conquests (should not be used at all).
For F2P, Jackson is the #1 cav (Plumer a close #2) and Moltke #6. You'll notice quite a difference if you compare Moltke to Jackson or Plumer.
For MacMahon, you should swap cover for inf commander as this skill is very useful in the 2 historical campaigns. Defence Formation is not a good choice as the 2 historical campaigns require killing enemies fast and not defending. Since you are also using MacArthur, may be wait until you get a replacement for MacArthur. You can make ambush/cover trigger all the time in your turn by using Torvesta's trick (need time and patience though).
