I assume those countries who are at neither war nor peace are neutral (can be at war or at peace, depending on what you do)?

I assume those countries who are at neither war nor peace are neutral (can be at war or at peace, depending on what you do)? Yes if not mentioned they are neutral. Of course relations can change during the conquest depending on your actions.

If you beat all factions not at peace with you before peace expires (I.E. Rome-Greece in Classical), what happens? Do you win, or does peace end? Or do you wait for turn 99?

No you still have to beat Rome. You can end the peace with Rome and attack them first or eventually they will end the peace and attack you.
