Looking up "General Camacho" on Wikipedia leads to Manuel Ávila Camacho and finding the predecessor is not too hard anymore.

He was a ruler of a state and the 4th ruler on the throne who shared the same name.
During his rule, he used proxies for war and expansion and his state was engaged in the 1st conflict against a rival. This rivalry would last for centuries.
His achievements did help successors further expand the state's influence westward.
He led many campaigns and conquered vast stretches of land.

More clues:
- His campaigns either led to or would contribute to his state's rise to power in the long run.
- His most renowned title of nobility is actually the first of its kind in its spelling. This made him the first in the line of rulers of who held that title.
- He was a controversial figure. Due to his activities in the realms of politics, society, warfare and personal life.

Is it Ivan IV, aka the Terrible? Definitely! You guessed his identity! I'm mildly annoyed at myself for giving in and looking it up, I guessed it was the first Tsar from your clues and given that I can only name about 5 and he's the earliest one of them...

This chap was born in 1906 and died in 1996.
A British Army officer in WWII rising to the rank of Lt. Colonel.
I'm pretty confident in saying he was the only officer to fight while armed with a longbow, broadsword and bagpipes!
