In the campaigns, there are objectives marked with a red circle. But if it is with a unit garrisoned inside the city, is there a way to find out if the objective means the unit or the city?

No there isn't until a unit is killed off you cannot be sure, but it is safe to assume that if there is a circle on a unit out on a city with a general, it is usally a unit that has to be eliminated

If it’s both, then you need a whole lot of artillery and hope that the general goes outside the city.

If said city & gen are targets, it's better that the target gen remained inside, so that we can kill him and take the city in one fell swoop. Otherwise, if the gen leaves, we have to hunt him down & attack the city which is most likely garrisoned by a fresh enemy unit.
